Medical school admission quota: the 2,000-place increase and its reversal
On 6 February 2024 the government raised the medical school admission quota, frozen at 3,058 since 2006, by 2,000 places a year from 2025 (5,058). Residents walked out; 4,567 students were finally recruited in 2025, then 3,058 in 2026. Under a framework act amended in April 2025, an independent estimation committee and the Health and Medical Policy Deliberation Committee replaced the plan in February 2026 with a smaller increase: 3,548 places in 2027, 3,671 in 2028 and 2029, then 3,871 with new schools in 2030 and 2031.

The 5,058 quota is not restored. Under the procedure of the 2025 act, the Health and Medical Policy Deliberation Committee set +490 places in 2027 and +613 a year in 2028-2031 over the 2024 quota (3,548 then 3,671), all through a regional doctor track, plus 200 places in new schools from 2030.
Stated objective
In 2024, add up to 10,000 doctors by 2035 to fill gaps in underserved regions and in essential care: about 5,000 doctors are needed to bring underserved areas to the national average, and many experts forecast a shortage of about 10,000 by 2035. In 2026, the new government’s objective is to train on average 668 more doctors a year from 2027 to work in regional, essential and public care.[1][19]

Main measures
- 2024 plan: quota raised by 2,000 places a year from 2025 (3,058 → 5,058), allocated mainly outside the capital region (82%), none in Seoul; regional flagship national universities brought to about 200 places; at least 60% of entrants in non-capital schools to come from the region.[1][4]
- 2025: universities could recruit 50 to 100% of their increase; 4,567 places were filled for a legal quota of 5,058.[5][7][19]
- 2026: a special article of the Higher Education Act Enforcement Decree (article 30-3) lets universities admit fewer students than their quota; 3,058 were recruited.[13][19]
- 2025 act: supply and demand estimation committees under the Minister of Health, up to 15 experts with no government members and a majority nominated by provider organisations, minutes published; the deliberation committee must respect their results when setting training numbers, from the 2027 physician intake.[10][9]
- 2027-2031: +490 then +613 places a year over the 2024 quota in the 32 schools outside Seoul, all through the regional doctor track (support during studies, 10 years of service in the region), with caps by type and size of university; a public medical graduate school and new regional schools add 100 places each from 2030.[19][20][22]

The debate
Citoyen takes no position: arguments are reported as their authors put them.
According to its supporters
- The quota had been frozen at 3,058 since 2006; according to the government in 2024, about 5,000 doctors are missing in underserved areas and many experts expect a shortage of about 10,000 by 2035 because of rapid ageing.[1]
- Doctors are unevenly spread: in 2023 Seoul had 3.61 doctors per 1,000 inhabitants (OECD average 3.7) against 1.80 in Gyeonggi and 1.89 in Incheon, according to the Ministry of Education.[4]
- The independent estimation committee, with a majority of members nominated by provider organisations, also found a shortage: 1,535 to 4,923 physicians in 2035 and 5,704 to 11,136 in 2040 in its base models.[17][14]
According to its opponents
- The Board of Audit and Inspection found that the 2035 shortage estimate behind the 2,000 figure (about 15,000 doctors) lacked logical consistency, and that the consultation with the Korean Medical Association and the deliberation committee’s review lacked procedural legitimacy.[21]
- Teaching capacity did not follow: according to the Board of Audit, places were allocated without site checks and with inconsistent criteria, Kangwon National University still had no anatomy lab for its extra students at the end of 2025, and eight new buildings will not be finished by December 2027 as planned.[21]
- University presidents and medical school deans asked in 2025 to go back to 3,058 recruits for 2026 to restore medical education after more than a year of conflict.[8][12]
- The Korean Medical Association contested the estimation committee’s method in January 2026 (time-series model, 2000-2024 data, no full-time-equivalent count of doctors, productivity assumptions); the committee replied that its estimate was the best available.[18]
